Richard Street, Bedford, PA 15522. You or your representative may personally pick up an application. Everyone who comes in during the above dates and times will receive an application. There is no need to arrive early or stand in line. You may return the completed application no earlier than 9 AM Tuesday, May 8, 2018. Only completed application will be accepted. Application returned before 9 AM Tuesday, May 8, 2018 will be discarded. Please contact us if assistance is needed to complete the application. Eligibility requirements apply.
